Key Responsibilities\n

• Conduct thorough evaluations and assessments of children and families to determine their specific needs.
\n
• Collaborate in the development and implementation of Individualized Family Service Plans (IFSPs) that tailor interventions to meet each child's unique developmental needs.
\n
• Regularly monitor and report on the outcomes of provided interventions as part of a multidisciplinary team.
\n
• Offer ongoing emotional support and educational resources to families, empowering them to support their child's growth.
\n
• Ensure compliance with all Federal and State regulations, as well as organizational policies, to uphold the highest standards of care.
\n
• Engage in evidence-based practices to foster rapport among children, families, and team members, creating a nurturing learning atmosphere.
\n
• Utilize the coaching model within natural environments to facilitate child development.
\n
• Advocate for family concerns and utilize their input in planning service delivery frequency based on developmental assessments.

Patient Population / Client Focus\n

This role focuses primarily on children under three years of age who are facing developmental delays or disabilities. You will engage with families from diverse backgrounds, providing tailored early intervention services that support the holistic development of their children. The essence of your work will be rooted in enhancing communication, social, cognitive, and emotional skills through personalized intervention strategies and supports.

Qualifications\n

• A Master's Degree in Social Work or a Doctorate of Social Work.

• Valid LCSW license certified by the Department of Public Health.
\n
• A valid driver’s license to facilitate home visits or community meetings as needed.
\n
• One year of experience working with early intervention services for children is strongly preferred.
\n
• Proficiency in adapting your approach to accommodate diverse learning styles and needs.

• Strong organizational, communication, and interpersonal skills, demonstrating respect for all stakeholders.

• Ability to work collaboratively within a team-oriented environment.
