Work Environment\n

This is a remote position that allows flexibility and adaptability while working from home. However, the LCSW will primarily engage with clients through virtual platforms, making flexibility in scheduling essential to accommodate client needs. Regular communication with the multidisciplinary team, including healthcare professionals and community service providers, ensures a collaborative approach to client care. Initial hours are Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed to meet client demands and ensure continuous support.

Key Responsibilities\n

• Monitor clients’ health and safety through regular virtual or in-home check-ins, addressing potential issues proactively.

• Facilitate family meetings to improve communication and resolve conflicts related to the care of senior clients.

• Maintain meticulous records of client interactions, assessments, and care plans in our electronic health record (EHR) system to comply with legal and ethical standards.

Qualifications\n

Compensation and Benefits\n

We offer a competitive annual salary ranging from $60,000 to $100,000, commensurate with experience and qualifications.
