Key Responsibilities\n

• Conduct comprehensive assessments and develop individualized care plans tailored to clients’ emotional and psychological needs.

• Provide counseling services that address the challenges and transitions involved in hospice care, including grief, loss, and adjustment issues for clients and their families.

• Utilize our user-friendly electronic health records (EHR) system to manage client information, billing, and scheduling seamlessly, with live support available for technical assistance.

• Engage with an intake and referral team to ensure clients are matched with appropriate services and support within our network.

• Collaborate with interdisciplinary teams to discuss care plans and patient progress, contributing to holistic treatment approaches.

• Maintain compliance with state and federal regulations while also upholding the ethical standards of social work practice.

• Participate in ongoing training opportunities to refine skills and stay informed about best practices in hospice and palliative care.

Patient Population / Client Focus\n

You will primarily work with individuals in hospice care, focusing on those coping with terminal illnesses and their families. Your role will be critical in providing emotional support, helping clients navigate their feelings about end-of-life, and facilitating discussions around life transitions. The goal is to enhance the quality of life for clients and provide resources and coping strategies that benefit both patients and their families during this challenging time.

Qualifications\n

• A Master's Degree in Social Work with an active LCSW license in the United States.
