Key Responsibilities\n

• Conduct comprehensive evaluations and psychosocial assessments to identify the needs of seniors and their families.

• Participate actively in the development of Individualized Family Service Plans (IFSP), ensuring that each plan addresses the specific needs and goals of our clients.

• Monitor and report on client progress and outcomes, collaborating with multidisciplinary team members to adjust interventions as necessary.

• Provide educational resources and emotional support to families, fostering a nurturing environment that prioritizes the well-being of the elder.

• Adhere to all Federal and State regulations, maintaining professional and ethical standards in all interactions.

• Utilize therapeutic techniques to build rapport, contributing to a supportive and trusting therapeutic relationship.

Patient Population / Client Focus\n

Your primary focus will be on seniors, a demographic that faces unique psychosocial challenges such as isolation, grief, and mental health issues. The role will involve direct interaction with this population and their families, navigating sensitive scenarios and providing tailored support. Your efforts will be essential in helping seniors maintain autonomy and improve their well-being through targeted interventions and resources.

Qualifications\n

• A Master’s Degree in Social Work (MSW) or a related field is required.

• Must hold a current Licensed Clinical Social Worker (LCSW) certification in the state of Florida.

• Strong organizational skills, with the ability to manage multiple cases simultaneously while meeting deadlines.

Compensation and Benefits\n
