You will work primarily in an outpatient setting, providing care to patients managing various mental health challenges. Key Responsibilities\n

• Conduct thorough diagnoses and treatment of mental, emotional, and behavioral disorders, adjusting care plans as needed to align with changes in patient status.

• Develop collaborative relationships with patients, their families, and other healthcare providers, ensuring a holistic approach to treatment and care coordination.

• Facilitate referrals to community resources, treatment programs, and support services to enhance patient well-being.
\n
• Work alongside a multi-disciplinary team to establish and execute comprehensive discharge plans, promoting continuity of care following treatment.

• Adhere to all federal, state, and local clinical program standards, including the Mental Health Practice Act, to maintain the highest quality of care.

• Evaluate daily priorities and workload by assessing new, ongoing, and discharged patient needs within your area of responsibility.
\n
• Engage in social work consultations, perform psychosocial assessments, and deliver supportive interventions based on individual client needs.
\n
• Effectively communicate with multiple patient care teams and external agencies to ensure seamless care delivery, timely discharges, and to troubleshoot potential delays or issues.
\n
• Provide debriefing support to staff members after adverse events, in conjunction with the Employee Assistance Program.
\n
• Act as an advocate for patients and families, facilitating effective communication with the care team and relevant external resources.
\n
• Identify patients at risk for avoidable delays or complex discharge needs, coordinating with care providers to determine optimal solutions.
\n

Patient Population / Client Focus\n

In this role, you will primarily work with a diverse population of individuals facing mental health challenges. This includes adolescents and adults dealing with anxiety, depression, trauma, and various other behavioral health issues. Your ability to build rapport with clients and assess their unique needs will play a critical role in their recovery and overall wellness.

Qualifications\n

• A Master's Degree in Social Work or a closely related field from an accredited institution, validated upon hiring.

• Current licensure as LCSW-C, LCMFT, LMFT, LCPC, or a licensed psychologist in the state of Maryland.

• Proficient understanding of community mental health resources available for patients.

• Strong written and verbal communication skills, with the ability to convey complex concepts in clear, accessible language.

• A proven ability to establish rapport with clients and foster a supportive therapeutic environment.

Preferred Experience\n

• Experience working in an outpatient mental health setting.

Compensation and Benefits\n

Unity Point Behavioral Services offers a competitive annual salary ranging from USD 90,000 to 100,000, commensurate with experience and qualifications. In addition, we provide a sign-on bonus of up to $1,000.
