Key Responsibilities

• Conduct comprehensive evaluations by reviewing patients' medical histories and performing specific assessments to inform your treatment plans.
• Clearly communicate evaluation findings and proposed treatment plans to patients and their families, ensuring they are actively involved in setting achievable rehabilitation goals.
• Implement individualized treatment protocols with proficiency, considering both patient evaluations and the continuously evolving healthcare landscape to maximize recovery outcomes.
• Collaborate with other healthcare professionals to ensure an integrated approach to patient care, documenting any changes in condition that may necessitate adjustments to planned therapies.
• Manage discharge planning effectively, ensuring continuity of care through detailed progress notes and follow-up plans that capture a comprehensive view of the patient's journey.
• Exhibit strong time management skills and uphold productivity standards while maintaining accurate scheduling and being punctual to all engagements.
• Participate in community-based marketing activities, promoting our services and the value of patient-centric care.
• Actively contribute to departmental goals and objectives while complying with all relevant governmental regulations and accreditations.
• Continue professional development through continuing education opportunities and serve as a mentor to aspiring clinicians.

Patient Population / Client Focus

Our patient population ranges from individuals recovering from surgeries to those managing chronic conditions affecting mobility, such as osteoarthritis, stroke, and neurological disorders. By providing tailored physical therapy directly in their homes, we aim to foster stronger partnerships with patients and their families, aid rehabilitation efforts, and significantly improve the overall patient experience.

Qualifications

• Doctorate Degree in Physical Therapy (DPT).
• A valid DPT license in the State of New Jersey.
• CPR certification from the American Heart Association, specifically for Healthcare Providers.
• Proficient understanding and experience in relevant treatment modalities.

Preferred Experience

• A minimum of 2 years of relevant clinical experience.
• Experience in home health care settings is advantageous but not mandatory.

Compensation and Benefits

The position offers a competitive salary range of $100,000 to $120,000 annually, accompanied by a comprehensive benefits package. This includes:
