Key Responsibilities

• Conduct thorough evaluations of clients' physical status, utilizing diagnostic tests to assess muscle, nerve, joint function, and determine the best course of treatment.
• Develop individualized treatment plans aimed at preventing disabilities and promoting recovery while considering the client's personal goals and lifestyle.
• Administer therapeutic exercises and treatments, including manual therapy, traction, and the application of various physical agents to relieve pain, improve mobility, and enhance overall well-being.
• Store, observe, and document client progress meticulously, updating care plans based on their response to therapy and collaborating with physicians and case managers for optimal outcomes.
• Instruct clients and their families on care strategies, usage of assistive devices, and home modification recommendations to support independence.
• Oversee Physical Therapy Assistants, ensuring they are delivering services in alignment with the prescribed care plan, and provide mentorship when necessary.
• Participate in interdisciplinary case conferences, contributing insights for diverse approaches to client treatment.
• Maintain accurate clinical documentation, ensuring compliance with state regulations and internal policies.
• Be a resource for clients and their families, answering inquiries and providing guidance throughout the therapy process.

Patient Population / Client Focus

The primary focus of this role is on a diverse clientele, including elderly individuals, post-operative patients, and those recovering from various physical ailments in home settings. Your efforts will directly influence their recovery journey, equipping them with resources, skills, and knowledge necessary for improving their physical health.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid and active Physical Therapist license in New Jersey devoid of any sanctions or disciplinary actions.
• Proven expertise in manual therapy techniques and the ability to create effective, tailored exercise programs.
• Familiarity with electronic medical records (EMR) systems, such as WebPT, for streamlined documentation.
• Strong knowledge of evidence-based physical therapy practices ensuring your techniques align with current research.

Preferred Experience

• Prior experience in home health care or similar settings is advantageous, as it highlights familiarity with unique challenges and patient-centered approaches in home therapy.
• Proven ability to work collaboratively in interdisciplinary teams, fostering a holistic treatment environment for clients.

Compensation and Benefits
