Key Responsibilities

• Conduct thorough evaluations of patients' physical capabilities and functional limitations during home visits.
• Design and implement individualized treatment plans that incorporate a variety of therapeutic techniques, including exercise regimens, hands-on manual therapy, and the use of specialized equipment.
• Regularly document treatment sessions, track progress, and assess patient responses to therapy, making necessary adjustments to treatment plans as needed.
• Collaborate with patients, families, and caregivers to provide education and training on assistive devices and daily living strategies to enhance mobility and independence.
• Maintain professional development by attending workshops and conferences, ensuring knowledge stays current with best practices in the field.
• Provide mentorship and guidance for new graduates entering the profession, fostering a supportive environment for continuing education.

Patient Population / Client Focus

You will work with a diverse patient population that includes children and adults recovering from various conditions such as orthopedic injuries, post-surgical recovery, neurological disorders, and chronic pain issues. Your ability to connect with patients emotionally and physically will be crucial, as their recovery often depends on building trust and comprehensive communication regarding their health goals.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid New Jersey State License in Physical Therapy.
• A minimum of two years of clinical experience in a similar role is mandatory.
• Solid communication skills, both verbal and written, to engage effectively with patients and their families.
• Proficiency in utilizing electronic health record systems for documentation and progress tracking.
• A flexible and adaptable nature to thrive in a fast-paced home care setting.

Preferred Experience

• Experience in home health care or community-based rehabilitation settings is preferred.
• Knowledge of assistive technology and its application in patient care.

Compensation and Benefits
