This position is based onsite, specifically tailored for home care settings within the community. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians to develop tailored treatment plans based on individual needs and medical history.
• Implement effective treatment strategies across various specialties including orthopedic, pediatric, neurologic, and sports medicine patients.
• Maintain clear and empathetic communication with patients and their families, fostering an understanding of the treatment process.
• Demonstrate exceptional customer service and contribute to the overall effectiveness of our team dynamics.
• Utilize advanced therapeutic techniques and procedures to facilitate recovery, ensuring alignment with the physician's plan of care.
• Consult with patients and caregivers, providing education on best practices for rehabilitation and self-care.
• Meet clinical utilization goals and other business objectives while striving for excellence in patient care delivery.

Patient Population / Client Focus

As a Home Care Physical Therapist at Convivial, you will work closely with a diverse patient population in the residential setting. This includes individuals recovering from surgeries, injuries, or chronic conditions. You will have the opportunity to affect real change in their lives by aiding in their rehabilitation, enhancing mobility, and improving overall health outcomes.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid state license as a Doctor of Physical Therapy by the Physical Therapy Examining Committee.
• At least two (2) years of clinical experience in a relevant setting.
• Current certification in CPR.
• Proficient in written and verbal communication, with solid computer skills for documentation and reporting.
• Ability to establish strong therapeutic relationships with patients, facilitating engagement and trust.

Preferred Experience

• Prior experience in home care or community-based rehabilitation settings is highly desired.
• Experience with a variety of patient demographics and conditions helps ensure versatility in treatment approaches.
• A track record of positive patient outcomes and high satisfaction ratings is preferred.

Compensation and Benefits

We offer a competitive compensation package with a salary range of USD 100,000 to 120,000 per year.
