Position Overview

Confluent Rehabilitation Physical Therapy is seeking a dedicated and skilled Physical Therapist to join our team in East Brunswick, New Jersey. Work Environment

This position is onsite, focusing on home care settings primarily in East Brunswick. Our physical therapists operate in various client homes, ensuring therapy services are not only medically effective but also tailored to fit the unique environment of each client. Key Responsibilities

• Conduct comprehensive evaluations of clients' physical abilities utilizing diagnostic tests focused on muscles, nerves, and joints.
• Design and implement effective treatment plans aimed at relieving pain, preventing disability, and restoring maximum muscle function.
• Supervise and collaborate with Physical Therapy Assistants to ensure the correct application of the treatment plan.
• Deliver therapeutic exercises and instruct clients and families in the rehabilitation process and the use of physical therapy devices.
• Administer traction, massage, and other physical agent treatments as prescribed.
• Make environmental recommendations to enhance client independence in their home.
• Regularly assess clients' progress and adapt treatment plans as necessary, documenting all findings in clinical records, including OASIS, progress notes, and discharge summaries.
• Lead interdisciplinary team discussions, contributing insights into the overall treatment plan for each client.
• Maintain open communication with clients, families, and other healthcare professionals to facilitate a unified approach to care.
• Stay compliant with documentation requirements and submissions per company policy.

Patient Population / Client Focus

Your role as a Physical Therapist will primarily involve working with adults and seniors who require support in restoring mobility, enhancing strength, and improving overall functionality following surgery, illness, or injury. The personalized approach we value ensures each client receives treatment tailored to their specific needs within the comfort of their own home.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid state license to practice in New Jersey, in good standing.
• Current CPR certification and a valid driver's license with automobile insurance.
• Excellent interpersonal and communication skills with an emphasis on patient and family engagement.
• A high level of professionalism and a creative approach to patient care.

Preferred Experience

We highly value candidates with at least two years of experience working as a Physical Therapist, particularly in acute care or home care settings. Your ability to navigate complex patient situations with compassion and skill will be pivotal in making a difference in our clients' lives.

Compensation and Benefits

The competitive salary range for this position is USD 100,000 to 120,000 annually, complemented by a comprehensive benefits package.
