Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate patients' mental health status, strengths, and specific needs.

• Implement evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), customized to each patient’s unique circumstances.

• Collaborate with a team of healthcare professionals—including psychiatrists and nurses—to develop and coordinate effective treatment plans.

• Assist patients and their families in understanding diagnoses, treatment options, and medication uses, fostering informed decision-making.

• Facilitate individual, group, and family therapy sessions using evidence-based modalities to promote emotional healing and support.

• Develop treatment plans in conjunction with the treatment team, ensuring alignment with patient goals and preferences.

• Advocate for patients navigate the complex mental health system, ensuring their rights and preferences are upheld.

• Facilitate therapeutic groups and psychoeducation sessions, and lead discharge planning meetings to optimize patient transitions.

Patient Population / Client Focus\n

This role focuses on supporting patients within our hospice care framework. You will work with individuals facing terminal illnesses, providing emotional support and therapeutic interventions to enhance their quality of life. The clientele may include diverse age groups and backgrounds, each presenting unique challenges requiring empathy, cultural sensitivity, and an understanding of end-of-life care.

• A Master’s degree in Social Work from an accredited institution is required.

• Current and active state licensure as a Licensed Independent Clinical Social Worker (LICSW) is mandatory.
