Key Responsibilities\n

• Conduct comprehensive bio-psychosocial assessments for clients dealing with mental health issues, substance abuse problems, and complex medical needs, specifically tailored for a primary care setting.

• Provide critical behavioral health support to a diverse patient population, including individuals, couples, and families, performing tasks such as counseling, intake assessments, diagnosing, treatment planning, and crisis intervention.

• Utilize electronic health record systems to accurately document mental health evaluations, progress notes, and suicide risk assessments, ensuring compliance with healthcare regulations.

• Serve as a liaison by connecting clients with essential community resources, including medication assistance programs and other support services that meet their holistic needs.

• Work actively with Quality Improvement teams to analyze data and improve service delivery processes, contributing to better patient outcomes.

In this role, you will provide care to patients experiencing complex emotional responses to their medical conditions, often associated with the end-of-life process. Qualifications\n

• Current and valid Licensed Clinical Social Worker (LCSW) license in the state of California.

• A Master’s degree in Social Work and a minimum of three years of post-licensure experience in a related field.

Compensation and Benefits\n
