Key Responsibilities\n

• Conduct comprehensive assessments to determine the mental health needs of patients and develop personalized treatment plans accordingly.

• Maintain regular communication with supervisors, direct care staff, family members, and other relevant parties regarding patient progress to ensure cohesive care strategies.

• Establish a tentative schedule that includes appointments, home visits, and progress monitoring observations to effectively track and advance patient goals.
\n
• Coordinate and perform home visits as needed to conduct essential in-home observations, chart reviews, and data assessments to support treatment plans.
\n
• Ensure that the implementation of behavioral health programs is effective and carried out according to established guidelines.
\n
• Participate actively in Interdisciplinary Team Meetings and collaborate with other professionals to enrich the patient care experience.
\n
• Document and complete monthly and quarterly progress notes to accurately reflect patient improvements.
\n
• Organize training sessions to ensure that staff and team members are well-versed in program implementation and documentation requirements.
\n
• Conduct follow-up interviews and additional home visits to provide ongoing support, ensuring that individual needs are continuously met.

• Act as a liaison with the community and mental health stakeholders to foster connections and promote developmental objectives.

• Assist in creating and refining training materials and procedures to enhance the effectiveness of programs offered to individuals.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted license as a Licensed Clinical Professional Counselor (LCPC) in the state of Illinois.

• A minimum of two years of professional counseling experience, ideally within a clinical context.

• Strong knowledge of legal and ethical standards pertinent to mental health practices.

• Exceptional interpersonal and communication skills, demonstrating an ability to build rapport with a diverse clientele.

• Proven capacity to work collaboratively within a healthcare team has been demonstrated.

• Familiarity with Electronic Medical Records (EMR) software and associated technologies is preferred.
