Key Responsibilities\n

• Collaborate with primary care providers to assess, diagnose, and manage mental health and psychosocial issues, enhancing the effectiveness of the overall care team.

• Conduct thorough patient intakes focused on diagnostic evaluations and functional assessments, crafting personalized therapy goals based on each client's unique needs.

• Provide timely mental health interventions for patients in crisis, ensuring they receive immediate support.

• Offer guidance to primary care practitioners regarding appropriate referrals and the necessity for specialized mental health services.

• Perform regular progress evaluations, assessing treatment efficacy and adjusting approaches as necessary for optimal patient outcomes.

• Support patients in developing relapse prevention strategies and assist them in identifying risk factors for deteriorating health.

• Coordinate mental health services, both within and outside of the primary care facility, to provide comprehensive care tailored to individual patient needs.

• Maintain accurate and up-to-date documentation in treatment charts, tracking progress and clinical notes meticulously.

• Foster open communication with primary care teams to ensure seamless care transitions and collaborative treatment planning.

• Educate patients on self-management techniques and the importance of adhering to medical treatments prescribed by their primary care providers.

Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from USD 90,000 to 105,000 annually, commensurate with experience and qualifications. In addition, we offer a competitive benefits package including healthcare, retirement plans, and paid time off.
