Key Responsibilities\n

• Conduct comprehensive patient intakes, focusing on diagnostic assessment and functional evaluations, before developing individualized therapy goals.

• Collaborate with primary care practitioners to identify patients who may require specialized mental health services, facilitating appropriate referrals.

• Monitor and document patient progress meticulously, making adjustments to treatment plans based on the efficacy of interventions.

• Develop and implement relapse prevention strategies tailored to individual patient needs.

• Engage with individuals to provide education on self-management skills and behavioral interventions that foster lifestyle changes aimed at improved mental health.

• Participate in multidisciplinary team meetings to enhance collaborative treatment planning and ensure consistent patient care.

• Maintain compliance with both Agency and Federally Qualified Health Center (FQHC) regulations while fulfilling weekly billable service productivity targets.

Patient Population / Client Focus\n

You will engage with a diverse clientele, ranging from children to adults, presenting various mental health challenges including anxiety, depression, trauma, and behavioral disorders. Qualifications\n

• A Master's Degree in Clinical Counseling is required.

Compensation and Benefits\n
