Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for clients, including individuals, adolescents, families, and groups, to determine their needs and appropriate interventions.

• Deliver graduate-level clinical social work and counseling interventions tailored to each client's unique situation and needs.

• Collaborate with interdisciplinary team members to enhance treatment planning through open communication and regular documentation.

• Demonstrate competency in addressing the physical, psychological, social, educational, and safety needs of patients across all age groups.

• Perform case management duties, which include consulting with community professionals and coordinating resources to ensure comprehensive care for assigned patients.

• Complete documentation in a timely and ethical manner as per departmental standards.

Compensation and Benefits\n
