Key Responsibilities\n

Your role will encompass a wide range of clinical responsibilities, including but not limited to:

• Conducting comprehensive psychosocial assessments and crafting individualized treatment plans based on the unique needs of each client.

• Diagnosing and treating a variety of mental health disorders while modifying care plans to align with any changes in patient status.

• Collaborating with multidisciplinary teams, including healthcare providers and community resources, to develop effective discharge plans and maintain continuity of care for each client.

• Providing support and crisis intervention, utilizing social work methodologies tailored to clients’ needs such as bereavement support and referrals to additional community services or treatment programs.

• Acting as an advocate for clients, ensuring open communication among all parties involved in the client’s care.

• Coordinating with care providers and leaders to identify patients at risk for delays in discharge or readmission, in order to develop effective strategies for their ongoing care.

Qualifications\n

To be considered for this position, candidates must possess the following qualifications:

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Psychologist.

Compensation and Benefits\n

Bright Health offers a competitive salary ranging from USD 90,000 to 100,000 annually, commensurate with experience and qualifications. Additionally, you may be eligible for a sign-on bonus of up to $1,000.
