Key Responsibilities\n

• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and create tailored treatment recommendations for clients.

• Collaborate closely with primary care providers to identify and manage mental health and psychosocial concerns, serving as a key member of the healthcare team.

• Provide brief, focused interventions for individuals requiring mental health support, utilizing evidence-based therapies and approaches.

• Maintain consistent follow-ups with patients to monitor their progress, adjusting treatment plans as necessary based on their evolving needs.

• Develop and implement relapse prevention plans aimed at sustaining clients' mental health stability.

• Document patient interactions, therapeutic interventions, and progress accurately in treatment charts, ensuring compliance with agency protocols and FQHC guidelines.

• Advise primary care providers on when patients should be referred to specialty mental health services versus those who can be managed in primary care.

• Educate clients on self-management strategies to empower them in their recovery journey, enhancing their overall quality of life.

• Engage with clinic management and fellow team members to improve behavioral health services and support program evaluation efforts.

• Actively promote behavioral health initiatives, helping patients develop healthier lifestyle choices and improve their overall health outcomes.

• Adhere to the highest standards of ethical conduct within the clinical profession.

• Undertake additional responsibilities as assigned to support team objectives and patient care.

Patient Population / Client Focus\n

As a counselor at The Core For Relationship Healing, you will work with a diverse clientele, including individuals facing a variety of mental health challenges ranging from anxiety and depression to relational conflicts and trauma. Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling from an accredited institution.

• Current and valid LPC (Licensed Professional Counselor) License, ensuring compliance with Arizona regulations.

• Strong foundational knowledge of mental health assessments, treatment planning, and therapeutic modalities, including DSM-IV diagnoses.

• Familiarity with the effects of mental and physical health issues on individuals and their families within the community context.

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 to 105,000 annually, commensurate with your experience and qualifications.
