Responsibilities
- Provide direct nursing care for residents in a long-term care setting, including assessments, monitoring, and appropriate interventions per care plans.
- Administer medications and treatments in accordance with physician orders and facility policies.
- Develop, implement, and update individualized nursing care plans; document all updates accurately.
- Monitor residents for changes in condition, respond promptly, and escalate to the supervising nurse as needed.
- Perform routine procedures (vital signs, wound care, injections) and assist with activities of daily living as required.
- Collaborate with interdisciplinary team members and families to ensure safe, patient-centered care and timely care coordination.

Requirements
- Active Registered Nurse license in New York in good standing.
- Graduation from an accredited RN program (ADN or BSN) and current RN status.
- BLS certification (American Heart Association) required.
- Prior LTC or skilled nursing experience preferred.
- Ability to work the listed shift and function effectively in a long-term care environment.
- Strong communication, documentation, and teamwork skills.
