Key Responsibilities\n

• Perform comprehensive assessments of clients' mental health needs through initial and ongoing evaluations, ensuring the creation of a personalized care plan.

• Maintain ongoing communication with supervisors, healthcare team members, and clients’ support systems to continuously assess client progress and adapt treatment plans as necessary.

• Schedule and conduct home visits, therapy sessions, and virtual check-ins, providing consistent follow-up to address any evolving mental health concerns.

• Develop and implement effective treatment plans that include therapeutic modalities tailored to individual client needs, incorporating evidence-based practices.

• Document client interactions meticulously in our EMR system to ensure accountability and allow for ongoing care coordination.

• Participate in regular Interdisciplinary Team Meetings to collaborate on treatment strategies, share insights, and enhance the overall effectiveness of care.

• Facilitate training sessions to ensure that team members are proficient in implementing program plans and adhering to documentation standards.

• Act as a liaison between clients and the broader mental health community, enhancing awareness of services and resources available.

Qualifications\n
