Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate clients’ needs, strengths, and support systems, ensuring a empathetic understanding of their current challenges.

• Design and implement individualized treatment plans that incorporate evidence-based practices, focusing on therapeutic interventions such as individual, group, and family therapy.

• Serve as a client advocate by researching and referring clients to community resources such as food assistance, childcare, and healthcare, facilitating improved access to essential services.

• Collaborate with an interdisciplinary treatment team, coordinating care for assigned patients and providing case management tailored to their unique situations.

• Respond effectively to crisis situations, including instances of child abuse and mental health emergencies, providing immediate support and intervention as needed.

• Regularly follow up with clients to monitor their progress, adjusting treatment plans as necessary to ensure optimal outcomes.

• Maintain detailed records of client interactions, assessments, and treatment progress within our electronic health record (EHR) system.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is required.
