Key Responsibilities\n

• Conduct thorough assessments of clients to understand their unique challenges and to tailor personalized treatment plans that support their mental and emotional health.

• Maintain clear and effective communication with supervisors, direct care staff, and family members concerning clients' treatment progress and necessary adjustments to care strategies.

• Draft a tentative schedule outlining appointments, home visits, and observations to keep the treatment process on track and to ensure regular check-ins.

• Coordinate and perform home visits as needed, ensuring compliance with in-home observations, chart reviews, and data collection to bolster effective care delivery.

• Oversee the implementation of therapeutic program plans, ensuring that interventions are executed appropriately and are yielding positive outcomes.

• Engage in Interdisciplinary Team Meetings and other critical discussions to foster collaborative care and to address any emerging issues in treatment.

• Generate detailed monthly and quarterly progress notes that reflect client advancements and areas needing attention.
\n
• Organize training sessions to enhance staff competency in implementing effective program plans and maintaining accurate documentation.

• Provide ongoing support through follow-up sessions, observations, and home visits to adapt treatment as necessary to meet evolving client needs.

• Act as a liaison with the broader mental health community to advocate for clients and enhance service delivery.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
