Key Responsibilities\n

• Conduct thorough and ongoing assessments to identify the mental health needs of patients, using standardized tools to inform treatment plans.

• Create and maintain a tentative schedule of appointments, home visits, and follow-ups to support client engagement effectively.

• Coordinate and conduct home visits when necessary, to gather insights through direct observations and staff interviews.

• Ensure accurate implementation of treatment plans, adapting strategies based on continuous evaluation of client response.

• Participate actively in Interdisciplinary Team Meetings to discuss case progress, ensure alignment, and strategize on care approaches.

• Complete detailed monthly and quarterly progress notes to document client outcomes and treatment efficacy.
\n
• Organize and conduct training for staff, enhancing their competencies in program implementation and documentation processes.
\n
• Conduct follow-up interviews and ongoing observations to provide continued support tailored to each client’s evolving needs.

• Maintain organized records and documentation to facilitate communication with involved parties within the mental health community.

• Act as a liaison between clients and external mental health resources to support their personal development and support pathways to wellness.

• Develop training materials and procedural documentation aimed at improving client programs and treatment quality.
