Key Responsibilities\n

• Conduct thorough assessments of patients, evaluating their mental health needs and safety status, and effectively identifying potential risks such as suicidal ideation or self-harm.

• Initiate and facilitate psychoeducational and process groups, fostering engagement and participation while ensuring a supportive environment for all clients.

• Develop and implement tailored treatment plans that reflect clients' unique circumstances, preferences, and therapeutic goals, revisiting these plans regularly in collaboration with the treatment team.

• Monitor and document patient progress, completing all necessary assessments and notes in a timely manner to meet compliance guidelines.

• Collaborate with a multidisciplinary team to assess and coordinate effective discharge planning for patients, ensuring continuity of care post-treatment.

• Provide one-on-one therapy sessions that focus on evidence-based interventions such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT).

• Engage in post-discharge follow-up to assess clients' status, reinforcing their support systems and ongoing mental wellness.

• Maintain clear and professional communication with clients and their loved ones, ensuring that all stakeholders are informed and actively involved in the therapeutic process.
