This is a full-time and part-time remote position, allowing you to provide critical mental health support to clients through telehealth services. You will be responsible for fostering emotional, mental, and physical growth in patients by delivering therapeutic interventions and implementing personalized care plans, all while adhering to Agency and Federally Qualified Health Center (FQHC) guidelines.

Key Responsibilities\n

• Conduct thorough patient intakes, performing diagnostic and functional evaluations to help create tailored treatment plans.

• Provide brief and focused interventions, specifically for patients requiring mental health services prior to referral for specialty care as needed.

• Carefully monitor and document each patient's progress, ensuring compliance with treatment goals and adjusting plans as necessary.

• Work collaboratively with primary care providers, offering insights on appropriate care settings and treatment approaches.
\n
• Develop and initiate relapse prevention strategies that assist patients in maintaining stability in their mental health.
\n
• Escalate concerns regarding “at risk” patients to appropriate care teams and help formulate action plans for their continued health.
\n
• Facilitate communication and coordination among external care providers, ensuring seamless patient experiences, with proper consent.
\n
• Provide educational resources and self-management strategies empowering patients to actively participate in their recovery.
\n
• Engage in program evaluation discussions with clinic management to optimize the effectiveness of behavioral services.

• Adhere to the Code of Ethical Conduct for Clinical Professionals and ensure all duties are fulfilled with professionalism and integrity.

You will be working with a diverse patient population, addressing various mental health issues that may arise within the community, including but not limited to anxiety disorders, depression, and family dynamics. Your role is crucial in guiding patients through their challenges and helping them achieve their treatment goals. Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling from an accredited program.

• A current Licensed Clinical Professional Counselor (LCPC) license.

Compensation and Benefits\n

The annual salary for this position ranges from USD 87,000 to 105,000, commensurate with experience and qualifications. We also offer a comprehensive benefits package, including health insurance, retirement plans, paid time off, and a sign-on bonus to attract top talent.
