Work Environment\n

This position operates in a hybrid format, primarily through virtual engagements which facilitate convenience for both our clients and team members. In addition to remote interactions, you will also engage in outpatient office visits as needed. This setting allows counselors to provide tailored support while fostering a bridge of communication among teams, clients, and families.

Key Responsibilities\n

• Conduct comprehensive assessments to accurately identify the needs and goals of patients in individual and group therapy settings.

• Facilitate clear communication among various stakeholders, including supervisors, healthcare team members, family members, and guardians about each client’s progress and treatment plan.

• Establish a roadmap of scheduled appointments and home visits, ensuring that plans align with best practices and therapeutic goals.

• Coordinate home visit protocols for in-home assessments, including chart reviews and data analysis, promoting a holistic understanding of client environments.

• Ensure the effective delivery of program plans and monitor adherence to set guidelines through thoughtful evaluation practices.

• Participate actively in Interdisciplinary Team Meetings, contributing insight and strategies to enhance patient outcomes.

• Document monthly and quarterly progress notes to track advancements and adjust treatment approaches as necessary.

• Lead training sessions to equip staff with the necessary skills to implement program plans proficiently.

• Engage in follow-up sessions, home visits, and observations to ensure ongoing support tailored to individual client needs.

• Maintain meticulous records, ensuring that all information is accessible to authorized parties, thereby enhancing transparency and care integration.
