Key Responsibilities\n

• Conduct thorough assessments of patients’ mental health needs, utilizing evidence-based tools and methodologies to inform treatment plans.

• Facilitate effective communication and collaboration with supervisors, direct care staff, families, and other key stakeholders to ensure continuity of care and evaluate individual progress.

• Provide detailed schedules of appointments, home visits, and observations to your supervisors, keeping them well-informed about caseload management and work advancement.

• Coordinate and perform home visits for in-home observations, intricate case reviews, and comprehensive data analysis to ensure individualized treatment plans are effectively executed.

• Participate actively in Interdisciplinary Team Meetings, contributing insights and feedback from a counseling perspective to enhance treatment approaches.
\n
• Maintain accurate and timely documentation, including monthly and quarterly progress notes, to meet compliance and regulatory standards.
\n
• Organize training sessions to certify staff on the implementation of program plans and therapeutic documentation, ensuring a high standard of care across the team.

• Provide wrap-around services, including follow-up interviews and ongoing support through home visits, tailored to each individual client's journey.

• Maintain detailed records and documentation to ensure transparency and accessibility of crucial information to relevant parties.

• Serve as a liaison within the mental health community and the public sector, advocating for effective developmental goals and resources.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
