Key Responsibilities\n

Your primary responsibilities will include:

• Conducting thorough psychosocial assessments to identify patients' emotional, social, and financial needs, offering tailored support plans.

• Providing individual and group counseling to address issues such as grief, adjustment to illness, and coping skills, facilitating patients' and families' emotional processing.

• Collaborating closely with medical teams to develop holistic care approaches and ensure that psychosocial factors are integrated into care plans.

• Coordinating with local community organizations to arrange post-discharge services, ensuring patients transition smoothly from active care to community-based support.

• Offering education and emotional support to patients and families regarding available treatment options, healthcare navigation, and available mental health resources.

• Engaging in crisis intervention and providing immediate emotional support during challenging times for patients and their loved ones.

• Participating in interdisciplinary rounds and contributing your social work expertise to enhance team-based patient care strategies.

• Maintaining accurate and timely documentation in compliance with state regulations and hospital policies, ensuring all records reflect the services provided.

• Contributing to quality improvement initiatives and hospital committees focused on enhancing patient satisfaction and improving care delivery.

Patient Population / Client Focus\n

This role focuses primarily on patients receiving hospice care, representing individuals facing terminal illnesses and their families. Qualifications\n

To qualify for this position, you must possess:

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
