Key Responsibilities\n

• Conduct thorough evaluations and assessments of clients to understand their unique social and emotional needs.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), ensuring tailored interventions that significantly enhance the care provided to patients.
\n
• Monitor and evaluate the outcomes of interventions as a key member of an interdisciplinary team, providing insights that drive improvements in patient care.
\n
• Deliver vital emotional and practical support to families, helping them manage the challenges associated with hospice care and ensuring that they feel empowered and informed.
\n
• Comply with all federal and state regulations, remaining abreast of changes in legislation that affect social work practices in hospice environments.
\n
• Build strong relationships with families, children, and multi-disciplinary team members, promoting a collaborative and healthy work atmosphere.
\n
• Utilize evidence-based practices and Natural Learning Environment Practices to develop and recommend strategies that align with the developmental needs of children.
\n
• Implement IFSPs for enrolled families, adjusting service frequency based on child development status along with the priorities and concerns expressed by families.
\n
• Offer families educational resources and practical skills that enhance their child’s development, facilitated through a coaching model and natural environment teaching approach.
\n
• Ensure the health and safety of clients and staff through strict adherence to organizational standards and guidelines.

Patient Population / Client Focus\n

The LCSW will work with families facing challenges due to disabilities or developmental delays in young children. Our focus is on helping children from birth to three years of age learn and develop essential physical, cognitive, social/emotional, and communication skills through supportive and individualized care. Your efforts will directly impact improving the quality of life for these children and support their families in difficult times.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Clinical Social Worker (LCSW) in the state of Texas, issued by the Department of Public Health.

• A valid driver’s license to facilitate travel when needed.
\n
• Ability to adapt to various learning styles and needs to provide personalized support to each child.
\n
• Demonstrated commitment to Natural Learning Environment Practices.
\n

Preferred Experience\n

• At least one year of experience working in early intervention services, particularly with children and families.

• Bilingual capabilities are encouraged to help effectively communicate with diverse families and communities.

• Proven organizational and time management skills, ensuring efficient handling of multiple cases while maintaining quality care.

• Exceptional communication skills, both verbal and written, to facilitate clear and effective interactions with families and colleagues.

Compensation and Benefits\n
