Key Responsibilities\n

As a Licensed Clinical Social Worker at our hospice center, your responsibilities will include:

• Conducting thorough evaluations and assessments to understand the emotional, social, and psychological needs of patients and their families.

• Participating in the development and implementation of Individualized Family Service Plans (IFSP) tailored to each family’s specific situation.

• Monitoring patient outcomes through continuous evaluation of care plans and adapting services as necessary to ensure the highest level of support.

• Providing compassionate support to families, equipping them with vital information and strategies that facilitate emotional and psychological well-being.

• Creating a safe, nurturing environment that promotes healthy interactions among children, parents, and team members.

• Utilizing evidence-based practices within the Natural Learning Environment framework to implement effective strategies for child development.

• Complying with all relevant Federal and State regulations, safeguarding the health and well-being of both clients and staff.

Patient Population / Client Focus\n

As part of our hospice center, you will focus on families dealing with the complex challenges that come with end-of-life transitions. This role involves working closely with both the patients, who may be facing terminal illnesses, and their families, who require support in coping with grief and making informed decisions. Compensation and Benefits\n
