Key Responsibilities\n

• Conduct comprehensive ongoing assessments to evaluate the mental health needs of patients, ensuring tailored service delivery.

• Facilitate communication with supervisors, care staff, family members, and guardians regarding each patient’s progress and treatment plan.

• Develop and provide a tentative schedule of appointments and home visits to supervisors, allowing for effective monitoring of patient progress.
\n
• Coordinate in-home visits to perform observations, conduct chart reviews, and gather data necessary for comprehensive evaluations.
\n
• Ensure that the implementation of each client’s program plan is effective and adheres to established protocols.
\n
• Participate actively in Interdisciplinary Team Meetings to collaboratively review patient cases and share insights for treatment advancements.
\n
• Regularly complete progress notes on a monthly and quarterly basis to document each patient's development and modify care plans as necessary.
\n
• Organize and facilitate training for staff to ensure competency in utilizing program plans and documentation processes.
\n
• Offer post-treatment follow-up interviews and observational visits to maintain ongoing support for clients’ evolving mental health needs.
\n
• Maintain thorough records to guarantee easy access to information for team members and stakeholders.
\n
• Act as a liaison between our clinic and the local mental health community, fostering collaboration and support for community development goals.

• Assist in developing training materials to enhance program delivery and operational efficiency.

Patient Population / Client Focus\n

In this role, you will provide support to a diverse clientele, including both adults and youth facing various mental health challenges. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• An active and unrestricted license as a Professional Counselor (LPC) in the state of Oregon.

• Minimum of two years of counseling experience, ideally in a clinical setting.

Compensation and Benefits\n
