Key Responsibilities\n

• Work closely with primary care providers to identify, treat, and manage mental health and psychosocial issues as part of an integrated healthcare team.

• Conduct thorough patient intakes, which include diagnostic evaluations and functional assessments, and develop evidence-based treatment plans tailored to individual patient needs.

• Provide focused and timely interventions for patients requiring mental health support, employing therapeutic techniques that are in line with Federal Qualified Health Center (FQHC) guidelines.

• Monitor the progress of patients across various treatment plans and maintain open communication with primary care providers for holistic patient management.
\n
• Develop relapse prevention strategies and help clients maintain stable functioning, preventing the escalation of mental health issues.
\n
• Document all patient interactions and progress in detail within treatment charts while ensuring that all entries are compliant with ethical guidelines and agency standards.

• Provide patients with self-management skills and educational resources to empower them in their recovery process and foster independence.
\n
• Collaborate with clinic management and other team members to evaluate program outcomes and propose enhancements to behavioral health services.

• Ensure adherence to the Code of Ethical Conduct for Clinical Professionals at all times.

• Take on additional duties as assigned to support organizational goals and the wellness of patients.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field is required.

• Current LPC license valid in the state of Pennsylvania.

• Ability to demonstrate effective communication skills and a compassionate approach to patient care.

• Understanding of the impact of physical and mental health disorders on patients and their families.

Preferred Experience\n

Compensation and Benefits\n

The annual salary for this position ranges between USD 87,000 and 105,000, commensurate with experience and qualifications.
