Work Environment

Key Responsibilities

• Conduct thorough patient evaluations, including reviewing medical history, performing necessary assessments, and documenting care plans with specific recommendations.
• Communicate evaluation findings and collaboratively develop treatment plans with patients and their families to align goals with patient expectations and preferences.
• Implement evidence-based treatment protocols tailored to individual patient presentations and the latest in health care practices, ensuring adherence to evolving standards.
• Consult with other healthcare professionals as needed to modify care plans based on changes in patients' conditions and document necessary adjustments systematically.
• Manage discharge planning effectively and provide comprehensive follow-up care with timely documentation that captures the patient's journey, ensuring clarity and compliance with healthcare regulations.
• Demonstrate strong organizational and time management skills to meet productivity targets while maintaining a positive relationship with colleagues and patients.
• Engage in continuous professional development, remaining updated with latest physical therapy practices and contributing to the educational growth of students and colleagues.
