Key Responsibilities\n

As a Licensed Clinical Social Worker at CHDC, your daily responsibilities include:

• Administering clinical behavioral health assessments to identify patients’ mental health needs and recommend appropriate interventions aligned with best practices.

• Facilitating navigation for mental health consumers through the complex physical and behavioral healthcare systems to ensure continuity of care.

• Providing education and resources to individuals around wellness, disease prevention, and a healthy lifestyle, thereby fostering engagement in their own health journey.

• Participating actively in interdisciplinary treatment team meetings, contributing insights that integrate behavioral health perspectives into comprehensive care plans.

• Ensuring timely and accurate reporting of crises and unusual incidents to appropriate medical leadership to maintain patient safety and quality care.

• Maintaining detailed clinical documentation in accordance with regulatory requirements and organizational policies to ensure high standards of care continuity.

• Engaging in Quality Improvement initiatives and participating in staff training sessions to enhance professional growth and organizational effectiveness.

• Utilizing evidence-based screening tools to assess and report healthcare needs and demographics of individuals in our care.

• Providing specialized support to patients with Opioid Use Disorder through collaboration with our dedicated Center of Excellence Team.

Qualifications\n

• A master’s degree in social work from an accredited institution is required.

• Current licensure as a Clinical Social Worker (LCSW) is mandatory for this position.

• A minimum of 2-3 years of relevant experience in behavioral health contexts is essential, preferably within medical settings such as primary or specialty care environments.
