Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify the mental health needs of clients, ensuring personalized care plans are developed.

• Create and monitor individualized treatment plans that include scheduled therapy sessions, follow-up assessments, and progress tracking.

• Perform home visits and virtual engagements as required to gather insights and evaluate client progress in their natural environments.

• Engage actively in Interdisciplinary Team Meetings to discuss patient outcomes and strategize treatment adjustments.

• Compile and submit detailed monthly and quarterly reports outlining client progress and program effectiveness.

• Organize training initiatives aimed at enhancing the team's skills in implementing mental health programs and utilizing documentation procedures effectively.

• Provide ongoing support through follow-up consultations, ensuring clients receive continual assistance tailored to their evolving needs.

• Maintain accurate and organized records to ensure information is readily accessible to authorized individuals.

Qualifications\n
