Key Responsibilities\n

• Conduct comprehensive evaluations of patients to assess their mental health needs and ensure appropriate care plans are developed and executed.

• Serve as a liaison among supervisors, direct care staff, family members, and guardians to facilitate effective communication around each client’s development.
\n
• Develop and monitor individualized counseling plans, incorporating in-home observations and follow-up assessments to track client progress.
\n
• Implement training sessions to enhance staff and team members’ competencies in following program plans and utilizing documentation tools effectively.
\n
• Maintain accurate documentation and records to ensure that all relevant information is accessible to authorized parties.
\n
• Participate in interdisciplinary team meetings to discuss case management and contribute to ongoing program improvements.
\n
• Provide ongoing support through follow-up interviews, home visits, and observations to ensure clients receive continued assistance.
\n
• Collaborate with the mental health community to achieve development goals for clients.
\n
• Generate monthly and quarterly progress reports on client outcomes and therapeutic effectiveness.
\n
• Assist in the creation of training materials to improve individual programs.

Qualifications\n
