Key Responsibilities\n

• Conduct comprehensive assessments to develop individualized treatment plans aligned with each client’s needs and therapeutic goals.

• Engage in effective communication with team members, supervisors, and the families of clients to discuss treatment progress and adjust plans as necessary.

• Maintain a schedule of appointments, home visits, and progress evaluations to ensure that clients receive ongoing support.
\n
• Facilitate home visits and collate data from various sources, including chart reviews and staff interviews, to ensure holistic understanding of client situations.
\n
• Collaborate in interdisciplinary team meetings to strategize on best practices in patient care and program implementation.
\n
• Document clinical findings and treatment progress in compliance with company standards and health regulations, ensuring information is readily available to custodians of care.
\n
• Design and deliver training sessions to enhance the skills of staff concerning program implementation and client documentation.
\n
• Act as a liaison between our organization and the broader mental health community, supporting efforts aimed at improving community health initiatives.

• Regularly provide follow-up care through interviews and home visits to assess the continuing needs of clients and adjust treatment plans accordingly.

Patient Population / Client Focus\n

In this role, you will work with a diverse population, including adults and youth facing various mental health challenges such as anxiety, depression, trauma, substance misuse, and family issues. Your expertise in family therapy, couples therapy, and trauma-informed care will be invaluable as you tailor your approach to meet the unique needs of each client. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

• An active and unrestricted license as a Professional Counselor (LPC) in New Jersey.

• A minimum of two years of professional counseling experience, particularly in a clinical context.

• A strong foundation in legal and ethical standards relevant to mental health counseling.
\n
• Exceptional interpersonal skills, with the ability to develop rapport and trust with clients from diverse backgrounds.

• Demonstrated ability to work collaboratively within a healthcare team.

• Proficient in utilizing Electronic Medical Records (EMR) software and other relevant technological tools.

Compensation and Benefits\n
