Key Responsibilities\n

• Conduct ongoing assessments to determine and address the needs of your clients effectively, utilizing evidence-based practices to tailor treatment plans.

• Facilitate effective communication among all stakeholders, including supervisors, direct care staff, family members, and other involved individuals, to ensure a cohesive approach to client care.

• Develop and provide a comprehensive schedule of appointments, home visits, and clinical observations, ensuring that all client care metrics are met.
\n
• Coordinate and conduct essential home visits to fulfill program evaluation requirements, including thorough chart reviews and staff interviews.
\n
• Implement and oversee program plans to ensure they meet the evolving needs of clients within the assigned caseload.
\n
• Participate in interdisciplinary team meetings, contributing insights and updates regarding client progress and program developments.
\n
• Document monthly and quarterly progress notes meticulously, tracking client advancements and adjusting treatment plans as necessary.
\n
• Organize and lead training sessions for staff members, ensuring understanding and compliance with the program plan and relevant documentation.
\n
• Manage follow-up interviews and home visits to maintain ongoing support and progression for each client's treatment plan.
\n
• Act as a liaison with the public and local mental health community, ensuring that you provide adequate support and resources to meet client goals.

• Assist in the creation and enhancement of training materials that promote efficient program implementation for the support of individuals.

Your work will focus on a diverse range of clients, including adults and youth, who are experiencing mental health concerns such as anxiety, depression, trauma, and substance use disorders. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• An active and unrestricted Professional Counselor (LPC) license within the state of Arizona is essential.
