Key Responsibilities\n

• Conduct comprehensive assessments to identify the individualized needs of clients, ensuring the delivery of targeted counseling interventions.

• Maintain effective communication between clients, their families, supervisors, and interdisciplinary team members to promote overall well-being and track progress.

• Prepare and share a tentative schedule outlining appointments, home visits, and observations to facilitate ongoing evaluation and progress monitoring.

• Coordinate and conduct home visits for in-home observations, focusing on adapting interventions to a comfortable environment for clients.
\n
• Implement and evaluate program plans to ensure they are effective and aligned with each client’s therapeutic goals.

• Participate actively in Interdisciplinary Team Meetings to enhance collaborative care planning across different specialties.
\n
• Document monthly and quarterly progress notes to track client developments and make necessary adjustments to care plans.
\n
• Organize and conduct training sessions to ensure all team members are well-equipped to implement the program plans and maintain accurate documentation.

• Provide continuous support through follow-up interviews and home visits tailored to the evolving needs of clients.

• Act as a liaison for the public and broader mental health community, assisting with developmental goals for programs and community outreach.

Qualifications\n

Compensation and Benefits\n

• Annual salary range of $90,000 to $110,000, commensurate with experience and availability.
