Key Responsibilities\n

• Conduct comprehensive assessments of clients to evaluate their mental health needs, ensuring that tailored treatment plans are developed and implemented.

• Maintain clear communication with various stakeholders, including supervisors, family members, and other professionals, to discuss client progress and necessary adjustments in treatment.

• Develop a tentative schedule, balancing home visits, client meetings, and analytical observations to provide effective support and enhance therapeutic outcomes.

• Coordinate and conduct home visits to gather vital information through observations, chart reviews, and staff interviews.
\n
• Ensure that all components of the program plan are executed effectively and that any issues are promptly addressed.
\n
• Participate in interdisciplinary team meetings and relevant discussions to contribute insights and collaborate on case management.
\n
• Document monthly and quarterly progress notes to track and evaluate client developments and program effectiveness.
\n
• Lead training sessions that ensure all staff are knowledgeable about program plans and documentation protocols.
\n
• Conduct follow-up interviews and observations to provide ongoing support tailored to each individual's evolving needs.

• Actively engage with the broader mental health community to share best practices and organizational goals.

• Assist in the refinement of training materials and procedures aimed at enhancing program efficiency and efficacy.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Pennsylvania.

Compensation and Benefits\n
