Key Responsibilities\n

• Conduct comprehensive ongoing assessments to understand the needs and progress of patients, tailoring therapy plans accordingly.

• Maintain effective communication with stakeholders, including supervisors and family members, to report on individual progress and encourage a collaborative approach to care.

• Provide detailed schedules of appointments, home visits, and follow-up sessions to supervisors, ensuring clarity in workload management.
\n
• Coordinate necessary home visits to meet specific patient requirements, conducting observations and chart reviews as needed.
\n
• Monitor and evaluate the implementation of individualized program plans, ensuring adherence to high standards of care and effective support for clients.
\n
• Participate actively in Interdisciplinary Team Meetings, bringing valuable insights into patient care and service enhancements.
\n
• Complete and document monthly and quarterly progress notes accurately, ensuring compliance with regulatory standards.
\n
• Lead training sessions for new staff members to enhance their understanding and capabilities in implementing therapy methodologies effectively.

• Provide ongoing support through follow-up interviews and visits, maintaining a strong connection with clients as they progress in their therapeutic journey.

• Maintain organized records to ensure all relevant information is accessible for healthcare decision-making and compliance.

• Act as a liaison within the mental health community, fostering partnerships to align development goals with available resources.

• Contribute to creating and improving training materials that aid in the proficiency of clinical practices and documentation.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field from an accredited institution.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas is required.

• A minimum of two years of counseling experience, particularly in a clinical environment, is mandatory.

• Demonstrated specialization in family therapy, couples therapy, trauma-informed care, or substance misuse is a plus.

• Strong familiarity with legal and ethical standards impacting mental health counseling practices.
