Key Responsibilities\n

• Conduct thorough ongoing assessments to determine the mental health needs of each patient, utilizing evidence-based tools and methodologies.

• Develop and maintain a tentative schedule for patient appointments, including home visits, to ensure consistent support and monitoring of treatment plans.

• Coordinate and execute home visits that may include observations, chart reviews, and data analysis to reinforce treatment goals.

• Engage actively in Interdisciplinary Team Meetings and similar forums to discuss patient care and program effectiveness.

• Document monthly and quarterly progress notes that reflect patient developments and therapeutic interventions.

• Organize training sessions to equip staff with competencies related to the implementation of treatment plans and documentation processes.
\n
• Facilitate follow-up interviews, home visits, and observations to support ongoing patient needs and reinforce positive outcomes.

• Maintain meticulous records that ensure information accessibility for all stakeholders involved in patient care.

• Act as a liaison with the public and mental health community, contributing to the development of supportive networks and resources for clients.

• Assist in creating and refining training materials to enhance program development and execution.

Patient Population / Client Focus\n

You will have the opportunity to work with a diverse array of clients, including both adults and youth, facing various mental health challenges. Qualifications\n

• Possession of an active and unrestricted LPC license within the state of Georgia.

• Annual salary range of USD 90,000–100,000, commensurate with experience and caseload capacity.
