This position allows for flexible scheduling and offers a competitive salary ranging from USD 90,000 to 100,000 annually, depending on experience and caseload availability.

Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the needs of each patient, utilizing a variety of evidence-based assessment tools and methodologies.

• Facilitate dialogue between supervisors, care staff, family members, and other stakeholders to provide updates on individual progress and collaboratively adjust treatment plans as necessary.
\n
• Prepare and maintain a tentative schedule for appointments, home visits, and progress monitoring to support each client’s treatment journey.
\n
• Arrange and conduct home visits as needed to perform observations, interviews, and data gathering that contribute to a holistic understanding of the client’s environment and support system.
\n
• Ensure that treatment plans are effectively implemented and meet the outlined goals, maintaining fidelity to the best practices in mental health counseling.
\n
• Engage actively in Interdisciplinary Team Meetings to discuss patient cases and strategize on care approaches, enhancing the collaborative care model.
\n
• Maintain meticulous progress notes, both monthly and quarterly, that track client improvements and provide insights into ongoing treatment efficacy.
\n
• Organize and lead training sessions that educate staff on program implementation and documentation to optimize best practices within the team.
\n
• Provide ongoing follow-up through interviews and home visits, ensuring continuous support and adjustment of care plans as needed.

• Serve as a liaison to the community, promoting mental health initiatives and connecting clients with additional resources.

• Contribute to the development of operational procedures aimed at enhancing program procedures and compliance.

Patient Population / Client Focus\n

This position involves working primarily with adults and youth facing a range of mental health challenges, including those related to stress management, relationship issues, trauma, and substance use. Qualifications\n

• A Master’s or Doctorate degree in Counseling, Psychology, or a closely related field is required.

• An active and unrestricted Professional Counselor (LPC) license to practice in Arizona.

• A minimum of 2 years of clinical counseling experience, preferably in a private practice or clinical context.

• Strong foundational knowledge of legal and ethical standards governing professional counseling.
\n
• Exceptional interpersonal and communication skills, with an ability to establish rapport and connect with a diverse clientele.

• Demonstrated ability to work collaboratively within a healthcare team to deliver coordinated care.

• Proficiency with Electronic Medical Record (EMR) software and familiarity with telehealth platforms.

Preferred Experience\n

• Specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance abuse counseling is highly preferred.
