Key Responsibilities\n

• Conduct comprehensive assessments of clients' mental health needs during initial and ongoing counseling sessions, ensuring tailored care plans.

• Maintain open communication with supervisors, family members, and other stakeholders regarding clients’ progress to ensure cohesive support and treatment.
\n
• Develop and share schedules of appointments, home visits, and other essential monitoring activities to track client advancement effectively.
\n
• Coordinate in-home visits for assessments, including chart reviews and interviews, to capture the full context of clients' lives and experiences.
\n
• Facilitate the successful implementation of treatment plans, ensuring adherence to established therapeutic protocols.
\n
• Engage actively in Interdisciplinary Team Meetings to collaborate on client care strategies and discuss treatment outcomes.
\n
• Document and complete monthly and quarterly progress notes to track client progress and treatment efficacy accurately.
\n
• Organize, participate in, and lead training sessions to enhance staff competencies on treatment plans and documentation practices.
\n
• Provide follow-up care through home visits and interviews to ensure ongoing support for clients and adjust treatment plans as necessary.

• Act as a public liaison and engage with the mental health community to foster partnerships and enhance service delivery.

You will work with a diverse range of clients, including both adults and youth, addressing issues such as trauma, substance misuse, and relationship challenges. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in Oregon.

• Minimum of two years of counseling experience, preferably in a clinical setting, working with individuals facing mental health challenges.

• Knowledge of and specialization in areas such as family therapy, couples therapy, trauma, or substance misuse is highly preferred.

• Strong understanding of the legal and ethical standards currently governing mental health practice.

• Exceptional interpersonal and communication skills to build rapport with diverse client populations.

• Demonstrable ability to work collaboratively within a healthcare team environment.

• Familiarity with electronic medical records (EMR) software and other digital tools relevant to mental health documentation.
