Key Responsibilities\n

• Conduct comprehensive assessments to effectively understand and meet the unique needs of the patients you serve.

• Maintain clear and ongoing communication with supervisors, direct care staff, family members, and relevant contacts concerning each client’s progress in treatment.
\n
• Develop and share a provisional schedule detailing appointments, home visits, and relevant observations to ensure accountability and monitor advancement in therapy.
\n
• Coordinate and carry out home visits, where necessary, to conduct in-home observations, chart reviews, and staff interviews.
\n
• Implementation of treatment plans, ensuring they are executed effectively and in alignment with client needs.
\n
• Participate in Interdisciplinary Team Meetings and other relevant meetings to contribute insights and collaboratively determine client strategies.
\n
• Fill out monthly and quarterly progress notes to accurately document client progress and changes in therapy.
\n
• Organize training sessions to ensure staff and team members are competent in implementing treatment plans and documentation practices.
\n
• Conduct follow-up interviews and in-home visits to provide continuous support tailored to the evolving needs of each individual.
\n
• Maintain meticulous records to ensure data is accessible to authorized parties and complies with legal and ethical standards.

• Act as a liaison, connecting with the public and other mental health professionals to promote awareness and development of mental health initiatives.

• Assist in developing training materials that enhance existing programs and services.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Possession of an active and unrestricted license as a Professional Counselor (LPC) in the state of Oregon.

• A minimum of two years of counseling experience is mandatory, preferably in a clinical setting.

• A background in family therapy, couples therapy, substance misuse, or trauma is highly desirable.

• A strong understanding of legal and ethical standards governing mental health practices is critical.
