Key Responsibilities\n

• Collaborate with a multidisciplinary team to assess patient needs and coordinate comprehensive discharge planning, ensuring readiness for transition.

• Conduct post-discharge follow-ups to evaluate ongoing patient progress and adjust treatment plans as necessary.
\n
• Lead psychoeducational and therapeutic process groups, creating a safe space for exploration and healing.
\n
• Develop individualized treatment plans and participate in regular treatment planning meetings for continuous assessment and improvement.
\n
• Conduct thorough safety assessments and monitor changes in patients’ conditions, reporting and responding to any safety concerns or risks.

• Utilize evidence-based interventions to address special precautions for high-risk patients, ensuring their safety and well-being.

• Complete all required assessments, clinical notes, and documentation promptly while adhering to organizational and regulatory compliance.

Patient Population / Client Focus\n

The Mental Health Counselor will be working primarily with adults and families facing various mental health challenges, including anxiety, depression, trauma, and relationship issues. Our clients come from diverse backgrounds, offering a rich tapestry of experiences and needs. As a key member of our team, you will be instrumental in creating an inclusive environment where all clients feel valued and understood.

Qualifications\n

• A master's degree in social work, marriage and family therapy, or a related field from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) in California.

• Proven ability to maintain appropriate boundaries with clients, creating a safe therapeutic environment.

• Strong communication skills, both written and verbal, to articulate complex concepts clearly and empathetically.

• Willingness to complete necessary background checks in adherence to regulatory standards.

• Experience working with a diverse client base is highly preferred.

Preferred Experience\n

While not mandatory, candidates with experience in community health settings or who have previously utilized teletherapy platforms will stand out. Familiarity with evidence-based practices and a background in integrated care or holistic therapy modalities will be viewed favorably.

Compensation and Benefits\n
