Although the work is carried out from home, you will maintain strong connections with clients and team members through virtual meetings and consultations. You will have the guidance and support of our Assistant Director, who will provide oversight and mentorship, ensuring that you continually make informed decisions regarding client care.

Key Responsibilities\n

• Conduct initial meetings with clients to assess their goals, preferences, and available options, laying the groundwork for personalized rehabilitation plans.

• Develop individualized rehabilitation plans that take into account clients’ educational backgrounds, aptitudes, physical capabilities, and aspirations for future employment, making necessary modifications throughout the care process.

• Collaborate closely with an integrated team of healthcare professionals, including physicians, occupational therapists, and psychologists, to design and implement rehabilitation programs that align with each client’s unique needs.
\n
• Maintain comprehensive case files for clients, ensuring thorough documentation of personal information, eligibility, services rendered, client interactions, and progress reports.
\n
• Monitor client progress consistently, evaluating achievements in relation to established goals and adjusting treatment plans as necessary for optimal success.
\n
• Engage actively with clients during their involvement in job training or occupational placement programs, providing ongoing support and promptly addressing any challenges that may arise.
\n
• Collaborate with clients’ families and support networks to ensure a holistic and effective approach to meeting client needs.
\n
• Prepare and submit timely analyses and documentation of client progress while making informed recommendations for additional resources as needed.

Patient Population / Client Focus\n

Your primary focus will be on clients experiencing various mental health and mood disorders, particularly those requiring outpatient support. By developing personalized care strategies and rehabilitation plans, you will empower clients to navigate their challenges and work towards achieving a higher quality of life. Qualifications\n

• A Master’s Degree in counseling, social work, psychology, or marriage and family therapy.

• Active Licensed Professional Counselor (LPC) licensure.

• Demonstrated experience in providing mental health treatment, particularly to clients with mental health or mood disorders in residential or outpatient environments.

• A collaborative mindset, showing a passion for teamwork to facilitate positive change in clients’ lives.

• Strong proficiency in clinical skills, including motivational interviewing, cognitive-behavioral therapy (CBT), and dialectical behavior therapy (DBT).
