Key Responsibilities\n

As a Licensed Clinical Social Worker at Mindful Walkway, your day-to-day responsibilities will include:

• Conducting comprehensive evaluations and assessments of children in hospice care to identify their specific developmental needs and challenges.

• Contributing to the development and implementation of Individualized Family Service Plans (IFSP) that outline tailored strategies for intervention and support.
\n
• Collaborating with a team of healthcare professionals to monitor and evaluate outcomes, ensuring that each child's care plan remains effective and responsive to their evolving needs.

• Providing emotional and practical support to families, equipping them with tools, information, and strategies to enhance their child's development.
\n
• Utilizing evidence-based practices and following natural learning environment principles to foster a supportive and engaging learning atmosphere.

• Ensuring compliance with all Federal and State regulations regarding child welfare and family support services.

Patient Population / Client Focus\n

This role primarily focuses on young children receiving hospice care, particularly those with disabilities or developmental delays. As you work closely with these children and their families, your interventions will aim to enhance their physical, cognitive, social/emotional, and communication skills within the framework of a compassionate and empathetic approach. Your expertise will help families navigate the complexities of grief and support them in celebrating their child's growth and unique journey.

Qualifications\n

The ideal candidate will have the following qualifications:

• A Master's Degree or Doctorate in Social Work from an accredited institution.

• Currently licensed as an LCSW by the Department of Public Health.

• A valid driver's license to facilitate travel for client visits as needed.
