Key Responsibilities\n

• As an LCSW reporting to the Social Work Manager, you will evaluate and treat a broad spectrum of psychosocial issues, utilizing individual clinical social work skills to aid patients and their families in navigating complex personal and environmental challenges.

• Conduct comprehensive bio-psychosocial assessments to identify barriers to treatment and enhance patient health status while documenting findings consistently within medical records as mandated by department standards.

• Develop detailed treatment plans in collaboration with our multidisciplinary team, employing evidence-based therapeutic modalities and continually analyzing treatment efficacy to drive positive patient outcomes.

• Provide education to patients and families, fostering an understanding of medical recommendations and social work interventions aimed at restoring optimal functioning in their daily lives.

• Facilitate integrated care by collaborating with medical staff to convey patients’ psychosocial backgrounds and needs, ensuring comprehensive care coordination and a superior patient experience.

• Participate in case management activities including the completion of case-related statistics and reports, and stay informed about community resources to advocate for the needs of our patient population.

• Engage in continuous professional development and maintain licensure through relevant training and educational activities that enhance your expertise in the field.

Patient Population / Client Focus\n

In this role, you will primarily serve patients receiving hospice care, many of whom face a range of emotional and psychological challenges associated with terminal diagnoses. Your expertise will be essential in assisting these individuals and their families as they navigate complex grief and acceptance processes, enhancing their overall well-being and quality of life.

Qualifications\n

• A Master's Degree in Social Work from an accredited graduate program and a current license from the State Board of Social Work Licensure are required.

• Licensure at the Licensed Clinical Social Work (LCSW) level is mandatory.

• Demonstrated proficiency in addressing the needs of diverse populations, particularly as it relates to aging processes and culturally relevant care practices.

Preferred Experience\n
