Full-Time Licensed Physical Therapist - Home Care Setting

Position Overview

In It Together Physical Therapy is seeking an experienced and compassionate Physical Therapist to join our dedicated team in New York City. This full-time role will primarily focus on providing home care services, allowing you to deliver high-quality rehabilitation treatment in the comfort of our patients' homes. Work Environment

This role will be carried out in a home care setting, where you will travel to various patients' residences throughout New York City. You will have a manageable caseload of approximately 6-7 patients per day, allowing you to dedicate adequate time and attention to each individual. Key Responsibilities

• Conducting thorough evaluations of patients in their homes to assess physical health, functional abilities, and rehabilitation needs.
• Developing, executing, and modifying customized treatment plans based on a thorough understanding of each patient's unique challenges and goals.
• Utilizing a variety of evidence-based techniques, such as therapeutic exercise, manual therapy, and neuromuscular re-education, to facilitate effective recovery.
• Educating patients and their families on best practices for injury prevention, proper body mechanics, and strategies for maintaining mobility and independence.
• Maintaining meticulous documentation of patient progress and modifying treatment plans as necessary to maximize healing and improve functional outcomes.
• Collaborating with our multidisciplinary team, including occupational therapists and orthopedic specialists, to ensure unified approaches to patient care.

Patient Population / Client Focus

You will primarily work with individuals recovering from musculoskeletal injuries, surgeries, or chronic conditions, as well as elderly patients requiring physical rehabilitation. Our patient-centered philosophy means you will tailor your approach based on each patient's specific needs, preferences, and progress, allowing for a personalized care experience.

Qualifications

• Must be a Licensed Physical Therapist in New York State.
• A Bachelor's degree in Physical Therapy is required; a Master's or Doctorate in Physical Therapy will be highly regarded.
• A minimum of 2 years of experience in a clinical setting is required, with a strong focus on outpatient care being beneficial.

Preferred Experience

• Experience working with elderly patients or individuals with orthopedic conditions is preferred.
• Prior experience in home care or a similar setting is an asset, though recent graduates are encouraged to apply.

Compensation and Benefits

This position offers a competitive salary range of $100,000 to $120,000, commensurate with experience.
